Fitness Center Access Request

This request enters into agreement that the person issued the access card on this form is a tenant or tenant employee and complies with the policies set in place for 2350 North Forest Road. The assigned access device is not transferable and must be surrendered when no longer being used by the issued party. Please note that a $15 fee applies for the issue of a replacement fob. Any fees are due at the time of the replacement request. Any accidental damage and all incidents must be reported.

In consideration of the opportunity to use the facilities and equipment on the premises, the undersigned agrees that he/she is solely liable for any personal injury or property damage arising out of said facilities and equipment, including without limitation any loss, cause or expense. The undersigned, having full disclosure of risks and dangers associated with the use of all facilities and equipment on the premises, and with full knowledge as to the absence of any representation or warranty regarding the condition of the property, hereby waives any right or cause of action, including but not limited to claims for personal injury or property damage, against any person or entity from which liability may or could accrue arising as a result of his/her use of the facilities and operation of the equipment of any type on the premises.

1. Prior to using the fitness center/sauna, all are required to sign the liability waiver form. 2. The fitness center/sauna is available to Hein 2350 North Forest Road, LLC employees/tenants only. Friends and family of employees/tenants are not allowed. 3. The fitness center may be used only by tenants of 2350 North Forest Road and their employees. Persons under the age of 18 ar e not permitted in this facility. Use equipment at your own risk. 4. The fitness club is not supervised. All equipment is used at your own risk. 5. Proper exercise attire and athletic shoes are required. (Specifically, t-shirts, sweats or shorts, and closed toed shoes. No open toe or hard sole shoes on equipment) 6. Food is not permitted. Closed, plastic beverage containers are permitted. 7. Must bring a workout towel and wipe off the equipment after each use. 8. Illegal substances, including alcoholic beverages, tobacco products, etc. are prohibited. 9. Avoid making loud sounds (banging weights, yelling, dropping hand weights, etc.) 10. Refrain from using loud, foul, slanderous, or confrontational language. 11. Profanity, offensive language, and offensive behaviors are prohibited. 12. Sexual harassment or offensive behavior of any kind will not be tolerated. 13. There is a 20-minute time limit on bikes and treadmill when others are waiting. 14. Allow other users to use resistance machine in between sets. 15. No music devices without headsets. 16. Re-rack weights and return all other accessories to their proper locations. 17. Any misconduct, carelessness, damage to equipment, and/or injuries are to be immediately reported. 18. Hein 2350 North Forest Road, LLC is not responsible for personal belongings lost or stolen. 19. Use of the fitness center is complimentary and an amenity for tenants and their employees. The utmost in courtesy should be observed with the use and orderly condition of the fitness center. 20. Abide by all rules listed here or posted in the building. 21. Non-compliance with these rules may result in loss of fitness room privileges. These guidelines exist for the safety and respect of everyone. Any behavior deemed inappropriate may result in loss of one’s access privileges.
